Our thanks to Ranald McIntyre for providing this for us. He arranged to get this scanned into a pdf file for us then had to split that into three to be able to email it to us. We thus thank him for going to all this work to make it available to our visitors.
Ranald worked for many years in the insurance industry dealing with the motor trade and became a bit of a Sherlock Holmes when investigating how an accident happened. One of these days we might persuade him to relate some of his stories :-)
